What should I bring to my first appointment?
In order for your surgeon to come up with the best treatment plan for you, most of the consult time should be spent examining you, reading x-rays, reviewing your health status, and discussing your options. Bringing the following items with you will ensure no time is lost just trying to gather information.
Please bring the following items:
1) A current list of all your medications - very important
2) A current list of your medical/health problems/issues - very important
3) A list of your previous surgeries
4) Fill out and bring in the medical history questionnaire we sent you (the yellow form) - very important
5) If you are not the biologic parent of a minor, or divorce has been involved, please bring in a copy of the legal custody/guardianship documents-very important
6) A copy of any/all advanced medical directives/wills, if you have any
7) If the patient is unable to give consent themselves, a copy of the care/guardianship documents must accompany the patient - very important
8) If you are less than 18 years old and have been emancipated in a legal court, we will require a copy of the those documents - very important
9) Copies of any/all medical/dental insurance cards you have - very important
10) If a community/church or other organization is willing to assist you financially for your treatment, we require a copy of that paperwork
11) Please call you physician, dentist, specialist, or other health care professional and ensure they have sent a referral via fax to our office - very important
12) Valid identification
