Patients WITH Dental and /or Medical Insurance
Our office has the experience working with many private insurance plans. It is important to bring your medical and dental insurance information. Some of your treatment may be eligible under medical and dental insurance. Your insurance coverage is a contract between you and your insurance company. Our office has no rights to information under the contract. Subsequently, it may be necessary for you to contact the insurance company directly to determine benefits. Please contact our insurance department if you need help.
Dental/Medical Insurance WE ARE contracted With
Consultation Appointments: You will be required to submit payment for any fee that your insurance deems ineligible prior to the service.
Treatment/Surgical Appointments: You will be required to submit your co-pay or deductible prior to service.
Dental/Medical Insurance WE ARE NOT Contracted With
If we are not contracted with your Medical/Dental insurance, it does not mean you have no coverage for the procedure you need. You will be required to pay for your treatment up front, then you will be reimbursed, after your procedure, by your insurance company.
After your consultation appointment, we will contact your insurance company and provide you with a treatment plan that reflects your coverage and your responsibility for the day of surgery.
Patients WITHOUT Dental and/or Medical Insurance
Payment of fees will be expected on the day of service.
Medicare
Our office is contracted with Medicare. Medicare coverage of dental procedures is poor. Medicare does cover some of the medical procedures and biopsies our surgeons perform. Patients are responsible for consult and/or surgery fees that are non-covered.
Payment of Fees
We accept cash, check, money orders, Visa, Master Card, Discover, and American Express for your convenience. Care Credit is available for those requiring a payment plan.
Interest Free Financial Assistance/Payment Plans Provided by Care Credit
Our office staff can help you fill out a Care Credit application and submit it by telephone. Once we have an approval, your treatment can begin. You pay the Care Credit Company a certain amount each month, so your fee can be spread out over a long period. The first 12 months are INTEREST FREE.
